How to make a FOIA request: If you would like to obtain information from a public body, you should begin by writing down a list of the information you are seeking. Then either use our Online FOIA Request Form, email a FOIA request, or prepare a letter to one of our FOIA officers. Your request should include your name, address, the date and a daytime phone number so that we may contact you if we have questions. Please describe the information you need with sufficient detail so that EIASE can find the requested records. EIASE requires that FOIA requests be in written form of either our Online Request Form, email, or letter.
